Overview

Thank you for taking the time to complete this short survey.

The National Mental Health Commission would like to understand a bit about how you, or someone you care for, or is in your care, experiences support for mental ill health and mental illness.

This survey is part of the National Mental Health Commission’s Connections project, which is about connecting with you to contribute to a national conversation about the future of mental health care, suicide prevention and wellbeing in Australia. We are developing a vision for a ten year plan through to 2030. Your input will help to inform our design for a more connected and well-functioning system.

Every Australian is welcome to be actively involved in this conversation and we encourage you to share the survey link with anyone who is interested. You can read more about the Connections project here.

What is involved?

The survey consists of six questions and will only take a few minutes to complete depending on how much information you would like to provide.

There will be a few questions that require you to select from a list of answers and some that require you to type your own answer in. At the end of the survey we will ask for information about you to help us better understand your needs and the needs of the community you live in. We would appreciate you completing this section, but each of these questions are optional, with a ‘prefer not to say’ option available.

You do not need to give your name to complete the survey, and your responses will be kept confidential and anonymous. You can save your responses and return to the survey at any time up until the closing date of 8 September 2019 using the save and return link provided. You will need to provide your email address if you choose to save and return. If you do not wish to provide your email address, you will not be able to save and return to your answers, and you will need to start the survey again.

What will my responses contribute to?

Your input will assist in developing a 2030 Vision for mental health, suicide prevention and wellbeing in Australia, as well as inform the priorities of the Commission for the next two years.

Privacy

The Commission takes your privacy seriously. All responses will be treated as confidential. No personally identifying information from your response will be disclosed or used for a purpose other than as set out above. 

The only people who will have access to your email address and other personally identifiable information that you may choose to provide are the Commission’s project team and the project staff of Primary Communications Pty Ltd, the Commission’s contractor. The Commission will not transfer or store your personal information offshore.

By including personal information and/or sensitive information in your response, you consent to the Commission collecting, using and disclosing that information in accordance with our obligations under the Privacy Act 1988 (Cth) including the APPs.
